Plenty of people go through years of denial on that count. With some it's due to upbringing, belief that homosexuality is wrong. Others simply aren't interested in women and are otherwise not sexually focused and thus don't notice their sexual attraction to men. In many cases these men have married, produced children and lived relatively "normal" lives until such time as they realize why they're unhappy. In Ethan's case I'd say he just never really noticed that he wasn't really in to women, especially since he responded when stimulated sexually... But then, a man will respond to almost ANYTHING that's rubbing up on his dick, whether it's to his taste or not. Sure, why not? Plenty of people go through years of denial on that count.
